The Laboratory of Hormone Optimization
Hormone ranges are based upon pooled data. “Normal” lab values are typically two standard deviations (95.4%) from a randomized mean. Ranges may be narrow, post-menopausal progesterone for example’s range is 0.1-0.8 ng/ml. Testosterone at one national lab is 264-916 ng/ml. (So, 275 ng/ml is “normal” and 900 ng/ml is “normal.”
Our goal is to optimize our patient’s health. That goal is impossible following those guidelines. I assure you if your patient’s total testosterone is 275 ng/ml he will have 90%+ symptoms of testosterone deficiency. If he is at 900ng/ml, he will have 90+% of the benefits of testosterone sufficiency. (The caveat here is we need to determine free testosterone and free androgen index-done for ou inside the video.)
Hormone levels should be centered around the median level of its acceptable range. The ideal net effect is levels in the upper 50th-75th percentile of the median range.
This lecture reviews “normal” and “optimal” (serum) laboratory values we need to obtain as part of our anti-aging program.

We cover diagnosis, and treatment strategies utilizing plant-based, bioidentical hormones and non-hormonal therapies. Lastly, we delve into the controversies surrounding (in) famous studies involving synthetic and natural hormones.

You will learn the physiologic role androgens play in men’s health. We discuss the functions of testosterone well beyond the role of sexual desire and performance. We identify the global therapeutic benefits of correcting andropause deficiencies, along with contraindications and side effects of excess, deficient, and insufficient testosterone, propose a diagnostic and treatment strategy and introduce the role of testosterone in the face of traumatic brain injury and the autistic spectrum.

You will learn, in layman’s terms, so you can clearly explain to patients the function of the thyroid gland using familiar analogies. We review the genesis of the thyroid hormones, the signs, and symptoms of thyroid disease, relevant laboratory studies. We introduce the concept of autoimmune thyroid issues and their role as a continuum throughout the body. We discuss diagnostic and treatment strategies well beyond the medical model of synthetic thyroid and nothing else. Diet, diagnostic targeting infectious disease, heavy metal toxicity, and the microbiome all play a role in thyroid dysfunction. We discuss the role of cortisol the stress hormone, and its effect on the thyroid and adrenal glands. Lastly, we propose treatment options including natural and synthetic medications, supplements, herbal remedies, then segue into lecture six, low dose naltrexone.

You will learn the pharmacology of naltrexone and low-dose naltrexone. We explore its roots as a potent anti-inflammatory and endorphin enhancing agent, its indications, contraindications, and side effects. We propose a dosage and treatment schedule for a variety of common issues, review the evidence-based uses amenable to LDN input, and discuss a variety of herbs and supplements which mimic the effects of low-dose naltrexone.

We review the main areas of inquiry, inflammation, nutrition, hormone optimization, mind, and body, to enact a successful BHRT program. We review the laboratory studies, either serum, saliva, or urine, including “normal” and “optimal” values to treat patients seeking relief successfully and safely from hormone deficiency symptoms. We devise treatment strategies for the ten vital hormones, and five essential lab values including studies involving inflammation, insulin resistance (two separate markers), nutrition, and immune function.
